Flight Reference Cards

CW observed that the Mk2 flight manual lacked any information on limitations and that the Flight Reference Cards were hardly complete; in fact, they were based primarily on the Chinook D Model, which is not fitted with FADEC. Squadron Leader David Morgan told the Board that the cards did not cover undemanded engine shutdowns, engine run-ups, spurious failure warnings or misleading or confusing cockpit indications. According to Morgan, what the cards did contain was confusing: ‘A number of emergency drills, in particular electrical and hydraulic, were poorly laid out and required the crew to be familiar with the drill to avoid confusion. The shortfall in the Chinook HC2 Flight Reference Cards was discussed with the crews during their conversion courses.’ Cook himself had made notes on his Cards about whether or not an ‘engine fail’ signal was likely to be genuine and Tapper expressed concern to his father about the Mk2, telling him that it would have to be checked out ‘very carefully’. Cook had also expressed concern to his father, telling him that the Mk2 was not ‘ready’ and that he and the other pilots had not had enough time flying to cope with all the (spurious?) warning signals that appeared.
